Transanal suture mucopexy was first described in 2012 by S. Chivate. A 56-year-old female with grade IV internal hemorrhoids is presented. Modified anorectal mucopexy and hemorrhoidopexy with transanal suture were performed. Through the PROMHISS (Patient Reported Outcome Measure-Haemorrhoidal Impact and Satisfaction Score) survey at 7 and 30 days after surgery, satisfactory clinical and aesthetic results are reported in this clinical case. The surgical procedure for hemorrhoids proposed by doctors Dr. Salinas and Dr. Cabrera is proposed as an effective, efficient, little painful and low-cost surgical option as well as with a very short learning curve. Modified anorectal mucopexy in combination with hemorrhoidopexy with transanal suture promises to be an accessible and easily reproducible procedure, with precise indications and another alternative in the management of hemorrhoidal pathology.
