Mesosigmoidoplasty associated with sigmoidopexy may be a non-resective surgical alternative for the prevention of sigmoid volvulus recurrence in elderly patients with high surgical risk. We present the case of a 93-year-old male with multiple comorbidity who was admitted for intestinal obstruction secondary to sigmoid volvulus. The diagnosis was established by plain abdominal radiography. After successful endoscopic devolvement and given the high risk of morbidity and mortality associated with colonic resection, a non-resective surgical approach consisting of mesosigmoidoplasty associated with sigmoidopexy was chosen. Clinical follow-up was performed on an outpatient basis for one year to evaluate recurrence and clinical evolution. The surgical procedure was performed without complications. The postoperative course was satisfactory, with resumption of bowel movements on the third day and hospital discharge on the sixth postoperative day. During the one-year follow-up, the patient remained asymptomatic, with no clinical evidence of recurrence of sigmoid volvulus or complications related to the procedure. Mesosigmoidoplasty associated with sigmoidopexy represents a safe and effective therapeutic alternative for the prevention of sigmoid volvulus recurrence in selected patients at high surgical risk.
