Pararectal gastrointestinal stromal tumors (GIST) are rare neoplasms that supose a surgical challenge. We report a 64-year-old female with a 53 × 30 mm intersphincteric mass. GIST diagnosis was confirmed via biopsy and CD117 positivity. Following six months of neoadjuvant imatinib, significant tumor shrinkage allowed for surgical excision through a direct perineal approach, preserving the anal sphincter. At 38 months of follow-up, the patient remains disease-free with normal continence. The perineal approach, supported by effective neoadjuvant therapy, represents a safe and functional surgical option for complex pelvic tumors.
